As we age, we need to be prepared for any situation. It's not enough to say you have a good doctor or health care provider anymore, you have to arm yourself with as much knowledge and education as you can. At Heart and mind, bundling our services together gives you the safety and comfort you need.
Dementia Bundle
The first thing that happens when we get a diagnosis is that we want to know what that means. We can see on TV, we may even have friends who get this devastating news. But what does this mean for YOU, what does this mean to those who love and care for you or your loved one?
Where do you begin? Should you start looking for placement? Should you look for someone to come help? When do you start to make your moves?
That's where Heart and Mind Training comes in, in that time of confusion and chaos, we bring the calm.
At Heart and Mind we will put you in our Alzheimer's Disease and Dementia Care Seminar, spend 2 hours in your home making sure that you or your loved one can safely Age in Place and teach you life-saving CPR/First aid so that you can cover all bases of health and safety.
As yours or your loved ones disease progresses we can look to adding a Nurse Navigator to assist you with doctors appointments and with navigating the very confusing medical care world. No matter what your background is, when it's our own life, we can become overwhelmed. We will make it all make sense.
Informational Bundle
In our single bundle package we ensure that you know what to expect with the new diagnosis.

In-home eval bundle
If you are planning on staying in the home as long as possible you will want to ensure that your home is ready for you to Age in Place.
This bundle includes:
- 8 Hour Alzheimer's Disease and Dementia Care Seminar for 1 person
- 1 Hour call with a Nurse to review options
- 2 Hour tour of your home to assist you in preparing to Age in Place safely
In-home health bundle
f you are planning on staying in the home as long as possible you will want to ensure that your home is ready for you to Age in Place and you have the knowledge to make sure everyone is safe.
This bundle includes:
- 8 Hour Alzheimer's Disease and Dementia Care Seminar for 1 person
- 1 Hour call with a Nurse to review options
- 2 Hour tour of your home to assist you in preparing to Age in Place safely
- 1 person credit for a CPR course with us
Family bundle
This is the premier bundle that ensures that everyone in the family is on the same page, has the same information and knows the plan moving forward. Families rely on each other in times of need, but if everyone has a different knowledge base, there will be confusion and eventual damage to the person with dementia.
Ensure that your family all has the same knowledge for the continuity of care of your loved one.
